Please Note: To place a fraud alert on your credit report with other credit reporting agencies you must contact them directly. Innovis does not share fraud alert information with any other credit reporting agency. Use the information below to contact the other credit reporting agencies.

Innovis provides you with two convenient ways to request an Initial Fraud Alert: by phone or by mail.
Phone: Call 1-800-540-2505. Representatives are available to assist you Monday through Friday, 8:00 a.m. - 8:00 p.m. ET. You will be asked to verify several pieces of information prior to fulfillment of your request. Once your identity has been verified, an Initial Fraud Alert will be placed on your Innovis Credit Report. If we are unable to complete identity verification by phone, you may be asked to submit your request by mail.
Mail: Complete the Fraud & Active Duty Alert Request Form and forward it to Innovis at the address provided below. Upon receipt and verification, an Initial Fraud Alert will be placed on your Innovis Credit Report for 90 days, beginning on the date of application.

If you place an Initial Fraud Alert, you may be entitled to receive one or more copies of your Innovis Credit Report at no cost.
Extended Fraud Alert (Requires an Identity Theft report)
To place an Extended Fraud Alert, which remains on your Innovis Credit Report for seven years, please submit your request in writing, along with a valid Identity Theft (ID) Report issued by a federal or local law enforcement agency. Complete the Fraud & Active Duty Alert Request Form and forward it to Innovis at the address provided below. Upon receipt and verification, an Extended Fraud Alert will be added to your Innovis Credit Report.
