A security freeze limits access to your Innovis Credit Report, preventing potential creditors from accessing your report without permission. By law, security freezes are available to consumers in about half the country. But Innovis gives all consumers the option to place security freezes on their Innovis Credit Reports (please refer to Security Freeze Fees for any applicable charges).

If you would like to place a security freeze on your Innovis Credit Report, please submit your request via certified or overnight mail.

Please complete the attached Security Freeze Request Form and mail it to Innovis at the address provided below:

Please include all of the following information:

  • Full name
  • Social Security Number
  • Date of birth (month/day/year)
  • Proof of current address, such as a current utility bill, lease arrangement or rental receipt
  • One copy of a government-issued identification card, such as a driver's license, state ID card, or military ID card
  • ID Theft Report (when applicable)

Once we place a security freeze on your Innovis Credit report, you will receive a confirmation letter that contains a 10-digit freeze confirmation number. If you have lost your 10-digit freeze confirmation number or a four-digit lift PIN and need a replacement please contact us at 1-800-540-2505

Please Note: To place a security freeze on your credit report with other credit reporting agencies you must contact them directly. Innovis does not share security freeze information with any other credit reporting agency. Use the information below to contact the other credit reporting agencies.

How to Lift a Security Freeze

You may lift a security freeze from your Innovis Credit Report for a specific credit grantor (grantor lift) or for a period of time (temporary lift). This will unlock your credit report to allow third party access. Please complete the attached Security Freeze Request Form and mail it to Innovis at the address provided below:

Please include all of the following information:

  • Full name
  • Social Security Number
  • Date of birth (month/day/year)
  • Proof of current address, such as a current utility bill, lease arrangement or rental receipt

  • 10-digit Freeze Confirmation Number (issued when we placed your freeze)

How to Remove a Security Freeze

To permanently remove a security freeze from your Innovis Credit Report, please complete the attached Security Freeze Request Form and mail it to Innovis at the address provided below. 

Note: Permanent removal should only be requested if you no longer desire to have your file frozen. Please refer to Security Freeze Fees for applicable charges.
